DOCUMENT PREPARATION
Commonly Requested Student Documents
Depending on your program and assigned requirements, Texas Rescue Med may request the documents listed below.
Identification & Education
- Driver’s license.
- High school diploma or GED.
- BLS CPR certification card.
- EMT certification for applicable advanced-program applicants.
Training & Clinical Eligibility
- ICS certificates.
- Traffic Incident Management certificate.
- Vaccination and immunization records.
- Health insurance card, when applicable.

Tips for a Successful Submission
Upload Clear, Readable Files
Make sure all text, names, dates, and other required details are visible. Check that document photos are not blurry, cropped, or obstructed.
Match Each Document to the Correct Field
Upload each document in the corresponding field. If one vaccination record contains multiple required immunizations, upload that same record in each applicable vaccination field.
Verify Your Documents Before Submission
Confirm that certifications and documentation are current where required, all necessary pages are included, and the information submitted is accurate.
